Agreement to Terms

By accessing and using Y2K Dentistry's services, you accept and agree to be bound by the terms and provision of this agreement. If you do not agree to abide by the above, please do not use this service.

Services

Y2K Dentistry provides comprehensive dental care services including but not limited to:

• General dentistry (cleanings, exams, fillings)
• Cosmetic dentistry (whitening, veneers, bonding)
• Restorative dentistry (crowns, bridges, dentures)
• Orthodontic treatment (braces, Invisalign)
• Dental implants and oral surgery
• Emergency dental care

Patient Responsibilities

• Provide accurate and complete medical and dental history
• Arrive on time for scheduled appointments
• Give at least 24 hours notice for appointment cancellations
• Follow post-treatment care instructions
• Maintain good oral hygiene between visits
• Pay for services according to agreed payment terms

Payment and Insurance

Payment is expected at the time of service unless other arrangements have been made. We accept cash, checks, and credit cards. For patients with dental insurance, we will file claims as a courtesy, but the patient remains responsible for all charges.

A service charge may be applied to returned checks and late payments.

Cancellation Policy

We require at least 24 hours advance notice for appointment cancellations. Failure to provide adequate notice may result in a cancellation fee. No-shows may be charged the full appointment fee.

Treatment Consent

By receiving treatment at Y2K Dentistry, you consent to the dental procedures performed. We will explain proposed treatments, but all treatment decisions ultimately rest with you. You have the right to refuse any treatment.

Limitation of Liability

Y2K Dentistry's liability shall be limited to the cost of the dental service provided. We are not liable for any indirect, incidental, or consequential damages.

Emergency Care

For dental emergencies during business hours, please call our office immediately. For after-hours emergencies, please call our emergency line or visit the nearest emergency room for severe conditions.

Modifications

Y2K Dentistry reserves the right to modify these terms at any time. Continued use of our services constitutes acceptance of any modifications.
